  BASH is an operational risk management (ORM) program. Step One of the ORM process is "Identify the Hazards". In order to do that, you need to know what bird species frequent your airfield. In order to do that, you must identify each and every carcass that is struck or suspected to have been struck at your airfield. If you insure that airfield managers, maintainers, and aircrew members remit each and every bird carcass for identification, you will do an effective job of hazard identification.   Privileged safety information refers to information that is exempt by statute or case law from disclosure outside the Air Force safety community. The military safety privilege is judicially recognized and protects the investigative process. The Air Force treats this information confidentially to ensure commanders quickly obtain accurate mishap information thereby promoting safety, combat readiness, and mission accomplishment. This is a briefing that you may use annually during your flight safety meeting to reassert the concept of privilege and the limitations on the use of privileged safety reports.

  The 439th Airlift Wing, an AFRC host unit, now employs this risk assessment matrix to determine their Phase II period of increased bird activity. It take into account the Bird Avoidance Model (BAM), the Avian Hazard Avoidance System (AHAS) and general airfield observations to ensure that all aspects of the risk are properly considered.

The 439th Airlift Wing Safety Office at westover air Reserve Base, Massachusetts developed this risk assessment for operating their C-5 aircraft. It includes several tiers of risk assessment. At each level, the items that are included include those items that can be controlled at that level. We invite you to benchmark from this superb example of operational risk management!
Microsoft Word Fatigue Post Mission Assessment
  In the ongoing effort to address aircrew fatigue risk, the 315th Safety Office at Charleston AFB, SC, led by Lt Col Debi Rieflin, developed a risk assessment matrix to assess aircrew fatigue after a mission. It is designed to account for acute, circadian and cumulative fatigue. It is designed to help an aircraft commander with an objective view of the crew’s overall fatigue risk at the end of the crew duty day – a time when accurate fatigue self-assessment may be compromised. Aircrews of the 315th AW use this information to identify and mitigate fatigue risk to follow-on missions. Aach aircraft commander completes this ORM worksheet at the end of each flying duty day during an off-station mission.
